Piriformis Syndrome vs Sciatica: How to Tell the Difference

One of the most common things I hear in my Denver acupuncture clinic is, “I think I have sciatica.”
Sometimes that is true. But many times what patients are actually dealing with is piriformis syndrome, and the distinction matters more than most people realize.
Both conditions can send pain, tingling, or numbness down the leg. The underlying cause, though, is completely different. Treating piriformis syndrome like disc-related sciatica can delay recovery or make symptoms worse. This is why sports and orthopedic acupuncture places so much emphasis on identifying the exact structure causing the irritation before any treatment begins.
What Separates Piriformis Syndrome From True Sciatica and Sciatic Nerve Pain
The biggest distinction is where the problem originates.
With true sciatica, the irritation comes from the spine, and sciatica starts in the low back near the hip before symptoms travel down the leg. Common structural causes include:
- A herniated disc
- Spinal stenosis
- Bone spurs
- Nerve root irritation at the lumbar spine
These spinal issues can create nerve compression and lead to painful symptoms.
Piriformis syndrome is a muscular problem. The piriformis sits deep in the glute and runs in close proximity to the sciatic nerve. When the muscle becomes dysfunctional, it can create compression or irritation around the nerve. The pain pattern can look and feel similar to sciatica, but the source is in the pelvis and hip, not the spine. Common risk factors for true sciatica include a sedentary lifestyle and poor posture.
A 2023 review in Hip & Pelvis noted that piriformis syndrome is a common differential diagnosis for sciatica, and that accurate identification requires looking beyond the spine to evaluate muscular and structural contributors along the entire nerve pathway.
Symptoms That Point More Toward Piriformis Syndrome
There are patterns in how piriformis syndrome presents that help differentiate it from a disc-related problem. Patients with piriformis syndrome tend to report:
- Deep, aching pain in the glute
- Pain when sitting, especially on hard surfaces
- Pain going up stairs or with hip rotation
- Tenderness deep in the buttock
- Radiating pain down the leg
One thing I notice consistently is that patients can point very specifically to a deep spot in the glute where the symptoms originate. That localization differs from typical disc-related sciatica symptoms, where symptoms often begin more centrally in the low back before traveling down. Sciatica is also common, and about 40% of people in the U.S. may experience it. It typically affects adults aged 30 to 50.
Prolonged sitting is also a major aggravating factor. Cyclists and desk workers make up a large portion of the piriformis syndrome cases I see, precisely because of the sustained hip position involved in both.
How We Differentiate the Two Conditions
Proper orthopedic testing is what separates a targeted treatment plan from a guess. It also includes a review of your medical history to help identify the root cause and any underlying issues. When needed, imaging can help assess structures that are harder to evaluate manually, including soft tissues.
Testing for Piriformis Syndrome
When piriformis syndrome is suspected, the evaluation focuses on:
- Manual muscle testing of the piriformis during external hip rotation
- Evaluating hip mechanics and range of motion
- Assessing the pectineus and other opposing muscles
- Pelvic balance and movement pattern analysis
Testing for Disc-Related Sciatica
For suspected spinal involvement, I use tests such as:
- Straight leg raise
- Slump testing
- Orthopedic spinal evaluation
The goal is to determine whether the nerve irritation is coming from the spine or from muscular dysfunction further down the chain. That distinction drives the entire treatment approach.
Why the Opposing Muscles Matter as Much as the Piriformis
This is one of the most clinically important points, and one of the most commonly overlooked.
Many patients assume the piriformis is simply too tight and needs aggressive stretching. In reality, the piriformis is often already in a lengthened and weakened position on the affected side. Stretching it more can worsen nerve irritation rather than reduce it.
The more productive question is: what is pulling the piriformis into that dysfunctional position in the first place?
The pectineus, a muscle in the front of the hip, is a frequent contributor. When it becomes tight, it creates internal rotational imbalance that continuously loads the piriformis. Until that imbalance is addressed, the piriformis will keep returning to the same dysfunctional state regardless of how much direct treatment it receives.
This is the kind of relationship that standard treatment approaches often miss, and why detailed movement assessment changes the outcome.
How Dry Needling and Acupuncture Points Treat Piriformis Syndrome
Acupuncture, dry needling, and electro-acupuncture are well suited as an acupuncture treatment for piriformis syndrome because they allow us to work directly within the involved muscles. This is different from treatments that only address symptoms at the surface.
Using dry needling and electro-acupuncture, the treatment works to support treating sciatica pain by changing the underlying mechanics that drive sciatic nerve pain:
- Reset dysfunctional muscle firing patterns to reduce muscle tension
- Reduce compression around the sciatic nerve to reduce inflammation
- Improve circulation within the affected muscles by improving blood flow and local blood flow
- Restore pelvic balance to calm pain signals
- Improve hip mechanics through the full movement chain to promote healing
Acupuncture works in part by stimulating nerves and supporting the nervous system.
It can also trigger endorphin release and the body’s natural painkillers to alleviate pain and reduce pain.
We also address surrounding muscles that contribute to the imbalance: glute medius, glute minimus, and the hip stabilizers. Treating only the piriformis in isolation may change symptoms short term, but effective pain management comes from addressing the full pattern. Working through the full pattern is what creates lasting change.

Common Mistakes That Slow Recovery
Two patterns consistently get in the way of recovery from piriformis syndrome.
Aggressive Stretching
Stretching the hamstring or glute can feel temporarily relieving, and patients often lean into it for that reason. But if the piriformis is already in a lengthened position, adding more stretch increases tension on the nerve rather than reducing it. Feeling better during a stretch does not mean the underlying mechanics are improving.
What Long-Term Recovery Actually Looks Like
Pressing directly into the piriformis with a foam roller or lacrosse ball is another common approach that can aggravate symptoms. The piriformis sits directly over the sciatic nerve. Deep, sustained pressure into that area can compress the nerve rather than release the muscle, and may slow recovery even when it feels productive in the moment.
Both of these mistakes reinforce why individualized assessment matters. The right intervention depends entirely on what the muscle is actually doing, not just where the pain is.
A Case That Illustrates the Pattern
One runner I treated had dealt with what she believed was sciatic pain for a long time. She had tried multiple approaches without sustained improvement. Her pain level changed over time but never fully resolved. On evaluation, it became clear that her piriformis was in a lengthened and weakened position, while the pectineus on the front of the hip was driving the entire imbalance.
Once we released the pectineus and worked through the pelvic balance, the piriformis normalized and the nerve irritation resolved. The presenting symptom was in the glute and leg. The evaluation showed the root cause of the sciatica pain was in the front of the hip, not just where the pain intensity was felt.
This kind of relationship between opposing muscles is something I see repeatedly. It is also why sports acupuncture approaches the hip and pelvis as a system rather than focusing on the site of pain alone.
Who Commonly Develops Piriformis Syndrome: Risk Factors
The patients I most commonly see with piriformis syndrome share certain patterns. Prolonged sitting and repetitive hip mechanics are the two biggest contributors. That puts the following groups at higher risk:
Cyclists, runners, and desk workers make up the majority of cases. Active adults with pelvic imbalance from years of sport or asymmetrical movement patterns are also frequently affected. The common thread is sustained or repetitive loading through the hip in a position that stresses the piriformis over time.
Getting the Right Diagnosis Changes Everything
Piriformis syndrome and sciatica can produce similar symptoms, but they are not the same problem and may call for different treatment options. Getting to the correct diagnosis is often the step that breaks the cycle for patients who have been managing symptoms without lasting relief. Acupuncture may be one of several common treatments or other treatments used depending on the diagnosis.
